We have appointments available on Saturdays for Families.
Vaccine costs are extra.

What to bring to your appointment

Valid identification cards such as a passport, driver's license, work permit, alien number, or any other government-issued photo identification.
Vaccination records, including COVID-19.
We will fill out the I-693 form with you in the office.

USCIS Immigration Medical Exam
Medical Exam & i693 form completed by a USCIS Approved Doctor
Vaccination review and verification.
I693 form and lab results copy
Sealed i-693 Form

Tests required by USCIS

Tuberculosis test (QuantiFeron test ) is required for all applicants 2 years of age and older
Syphilis test is required for all applicants ages 18-44
Gonorrhea test is required for all applicants ages 18-24

Vaccines/ Immunization records

We will help you once you arrive, so do not worry if you don’t have everything with you. Try to bring your vaccine records, including but not limited to Covid vaccines. If you don't have records of previous vaccinations, we can check your blood to determine your vaccination history (additional fees apply).
